Instructions to Apply for District Fee Waiver 1. Food Stamps: If your family is currently receiving Food Stamps, please check this box, and then complete Items 2, 3 and 6. 2. SNAP/TANF: You must print your current SNAP or TANF # on this line, not Link card. 3. Students: List the names of all students, their grades, school and ID#. If the student is a Foster Child, please place a check mark in that box and proceed to Line 6. Please note: If there are other children in the family who are NOT Foster children, then you must complete the entire Application. a. Categorically Eligible: If the child is Homeless, Migrant or a Runaway, please check the appropriate box. For a homeless student you must contact the District Homeless Coordinator at 630-299-7906. 4. Household: You must list ALL household members including the students named in Item #3. For each person who receives an income, please put the amount that they receive (BEFORE Taxes and Deductions) and how often they get paid. (Please see example). a. Earnings/Wages/Salary for each job: i. Current paycheck stub for each working member of Household which shows frequency of payment. If this is not shown, then you must provide one months’ worth of paystubs; ii. Letter from employer stating Gross Wages and how often they are paid; iii. Copy of Page 1 of most recent IRS Form 1040; or iv. Benefit letter from a Welfare Agency b. Welfare/Child Support/Alimony: i. Government aid benefit letter; ii. Statement of purpose of benefit; iii. Court decree, agreement or copies of checks received; iv. State disbursement website print out. c. Pensions/Retirement/Social Security: i. Social Security retirement benefit letter; ii. Statement of Supplement Security benefits (SS) received for each person; iii. Pension award letter; or iv. Veterans Affairs benefit letter/Union Strike benefit letter. d. Worker’s Compensation/Unemployment/Disability: i. Notice of eligibility from State Unemployment Office; ii. Disability award letter or Check stub; or iii. Letter from Worker’s Compensation. e. Frequency of Payment: i. Weekly; ii. Biweekly (every other week); iii. Twice a month; iv. Monthly f. No Income: 1. If you have no income, send a brief letter explaining how you provide food, clothing and housing for your household. 5. People in Household—Write in the total number of people in the Household. Make sure they are all listed in Item #4. 6. Signature: Please date, print name and sign name stating that all information on the Application is true and correct and that you are aware that supplying false information is a felony in the State of Illinois. 8/2014

Familia: debe listar a TODOS los miembros en la familia incluyendo a los estudiantes mencionados en el artículo #3. Por cada persona que reciba un ingreso, por favor coloque la cantidad que recibe (ANTES de impuestos y deducciones) y con qué frecuencia son remunerados. (Por favor, observe el ejemplo). a. Ingresos/Sueldos/Salarios por cada empleo: i. Talón de pago reciente por cada miembro de la familia que trabaje, el cual muestre la frecuencia del pago. Si no se muestra, deberá proveer el valor de los recibos del pago de un mes. ii. Carta del empleador estableciendo el sueldo bruto y con qué frecuencia es remunerado. iii. Copia de la página 1 del formulario IRS 1040 más reciente; o iv. Carta de beneficio de una agencia de asistencia pública. b. Asistencia pública/Manutención infantil/Pensión alimenticia: i. Carta de beneficio sobre asistencia del gobierno; ii. Declaración del propósito del beneficio; iii. Decreto judicial, acuerdo o copias de cheques recibidos; iv. Formulario del sitio Web sobre el desembolso estatal. c. Pensiones/Jubilación/Seguridad social: i. Carta de beneficios de jubilación del Seguro social; ii. Declaración de beneficios de Seguridad suplementaria recibida por cada persona; iii. Carta sobre otorgamiento de pensión; o iv. Carta de beneficio sobre asuntos veteranos/carta de beneficio sobre huelga del sindicato. d. Compensación laboral/Desempleo/Discapacidad: i. Aviso de elegibilidad de la oficina estatal de desempleo; ii. Carta de otorgamiento de discapacidad o talón de cheque; o iii. Carta de compensación laboral. e. Frecuencia de pago: i. Semanalmente; ii. Cada dos semanas (cada otra semana); iii. Dos veces al mes; iv. Mensualmente f. Sin ingreso: 1. Si usted no tiene ingreso, envíe una breve carta explicando cómo provee alimento, ropa y casa para su familia. 11. Personas en la familia—Escriba el número total de personas en el hogar. Asegúrese de listarlos a todos en el artículo #4. 12. Firma: Por favor, escriba la fecha, nombre escrito y firma confirmando que toda la información en la solicitud es verdadera y correcta y que usted tiene conocimiento que el proveer información falsa es un delito en el estado de Illinois. 8/2014
